Home Health/Hospice Team
Patient comfort is the highest priority for Home Health/Hospice caregivers. The Home Health/Hospice team at Elmhurst Memorial Healthcare consists of healthcare professionals who continually coordinate all patient care to maximize patient outcomes.
Patients are cared for by registered nurses, licensed physical and occupational therapists, speech-language pathologists and certified home health aides. Medical social workers also are available to direct families to financial and community resources, as well as provide short-term counseling for those who are adjusting to chronic illness.
