What You Need to Bring
Whether you visit the Immediate Care Center for an ear infection, sore throat or sprained wrist, you and your family will receive the same high-quality, personal care that you have come to expect from Elmhurst Memorial Healthcare.
It is important to remember the following items before visiting the Immediate Care Center:
- A copy of your most recent insurance card (some insurance coverage varies, so we encourage you to check with your carrier about coverage for immediate care visits)
- Driver’s license or other picture ID
- Names and dosages of any prescription or over-the-counter medications that you are currently taking
